Insurance and Payment Policy

How we save you money

Most of our treatments and procedures can be done right in our office in one of our state of the art procedure/ surgical suites. This means that you will not have a secondary charge for a facility fee, unlike many other physician practices. We save you money without compromising quality of care! When you have your procedure, at one of our four convenient locations, you also get amenities such as free parking, wheelchair accessible entry, and easy check in. In addition, we also offer a discount for a cash pay option plan, please call one of our friendly staff to inquire. Payment Policy

Payment of ALL copays, deductibles, and/or coinsurance are requested at the time services are rendered. As a service to you, our office will verify insurance coverage in advance and in accordance with your insurance contract. We do file insurance as a courtesy for our patients, but the responsibility of the bill does rest with the patient. This office cannot accept responsibility for collecting your insurance claim or for negotiating a settlement on a disputed claim.

Health Insurance

We participate in most governmentand commercial HMO-PPO plans and we accept assignment on Medicare patients.

Patients can check with their insurance company to verify our participation and to check on their policies regarding referrals, precertification requirements, co-pays, deductibles, services covered, etc.

Feel free to check with us regarding participation as well. Co-pays must be paid at the time services are rendered, per both the patients and our contract with the insurance company. Primary and secondary insurance filing will be completed at no charge.

Surgical and Procedure Fees

You may receive a call regarding your insurance benefits prior to a surgery or procedure from both our office and the facility where your surgery will be performed ( if not performed in our office). We make every effort to make you aware of estimated amount costs and require payment of the estimated prior to surgery. Uninsured patients will need to discuss estimated costs with the bookkeeping office in advance of surgery. For your convenience we accept MasterCard, Visa, and Discover.

Cancellation Policy

Because our appointments are booked several days in advance, we ask that you call us no later than 24 hours in advance to cancel your appointment. Appointment canceled after 24 hours (or one business day), will be subjected to a $35 cancellation fee. This fee is patient responsibility and is not billed to insurance.
